I release Ellen Wahl and Tai Chi With Ellen LLC harmless from any and all liability arising out of any personal injuries or damages, foreseeable or unforeseeable, which may occur as a result of participation in any individual or group instruction which includes private lessons, classes, workshops or retreats sponsored by Ellen Wahl and Tai Chi With Ellen LLC. Furthermore, I understand that if I have injuries or concerns I want Ellen Wahl and Tai Chi With Ellen LLC to be aware of, it is my responsibility to make them aware.
I hereby declare myself physically and mentally sound and capable of participation in any individual or group instruction which includes private lessons, classes, workshops, conferences or retreats.
